Mr Duff aged about Seventy extremely Corpulent, has most of his life
been subject to Slight Colics & disorders in the Stomach. He has always been
accustomed to live fully & richly, but never was a hard Drinker. He has
always been an active man & used as much Excercise as his great bulk would
allow him. For many years past he has been Subject to the Piles which
usually discharged a good deall of Blood & were of great service to him, both
in relieving his Colicky Complaints, & in removing a lethargic disposition which
often made him drop asleep in Company. For severall years past his appetite
& Digestion have been declining
& he has lost much of his flesh. He has been
used to take a Journey into England once in two or three years which commonly
agred extremely well with him. For this year past his appetite has been quite
Decayed
& he has eat little solid food. About five weeks ago he was seized
with a Severe fit of the Colic, attended with Costiveness & a Vomiting of tough
ropy Phlegm
, but without any feaverish Symptoms. Being averse to Injections
he got some Sacred Tincture which operated well & gave him relief from the Pain.
This however recurred in a few hours, but upon vomiting some Ropy Phelgm tur↑ned ↑

easier. The Pain has always been in the Pit of his Stomach & from that to his
Navell. It continued to recurr in this way for severall days, sometimes with great
Violence. He got two Vomits of [Hippocacuan?] which brought up much Phlegm
& Putrid bile. This gave relief for a day or two, but still the pains & vomiting
recurred. His belly was kept open by the Sacred tincture. Warm fomentations
or warm sand applied to his belly gave him ↑ease↑ but he always gott the most
effectuall relief from Opiates which the Violence of the pain & want of Sleep
occasioned by it, made necessary. The Opiate always gave relief & never seemed
to produce any bad Effect. Tho his Complaints always recurred yet they abated
gradually of their Violence & as soon as he could travell he was sent to his Country ↑Seat↑
& advised to use as much excercise on horseback or a chaise as he could bear. He
has been there these three Weeks past & is greatly better on the whole, but is never
a day quite free from pain. He takes some Panada with a little Wine in it in
the morning which agrees much better with him than tea. At Dinner he takes a
little Animall food (with reluctance) drinks Claret & Water & takes some toasted
bread dipped in it. His Supper is some warm Claret & toasted bread. This
always puts off any sickness or slight pain & gives him a good flow of Spirits.
His bad times are from twelve to two in the forenoon & from six to eight at

night, at which times he has pains in the Pit of his Stomach, sickness & reaching
which brings up much Phlegm & a great Deall of Wind. Chamamele tea sits
easy on his Stomach & often carrys off the Sickness. He is two hours on horseback
every day, but the motion of the chaise raises the sickness & vomiting which makes
him so much afraid of it that he has only tryed it three or four times. He is
always best in warm dry weather, & is easy when warm in bed. He frequently belches
up sour Stuff
from his Stomach, & when this is the Case he takes some Magnesia alba
which operates gently with him by Stool without occasioning any grypes. When
threatned with pain at bedtime, twelve or fifteen drops of Laudanum makes him
easy & procures him a good night. From the beginning of his disorder whenever
the pain left him, he had not the lest complaint, & till twelve o clock forenoon he
he is perfectly well, & from dinner till six at night he has frequently been threatned
with the Piles in this last Illness, but they have not advanced, & have blooded but
very little
. He makes less Urine than Usuall, but has no pain in passing it.
