The Documents have been linked together to form Cases. This has been done on on the basis of linking together all documents making reference to that individual patient (however trivial). Letters mentioning more than one patient are therefore linked to more than one Case. Where there is a long gap in time between a patient consulting Cullen, two distinct Cases have been created (but cross-referenced). Where an isolated enquiry may concern an earlier patient but there is not enough information to be certain it is the same person, then it is treated as a distinct Case.
