Of a middle size, and phlegmatic temperament.


His complaints are, at present,
cough attended by Dyspnœa and the expectoration
of a thick, viscid, phlegm
, which falling on the top
of the Pharynx, frequently causes vomiting;
anasarcous swellings of his Legs and Thighs:
rather tense feel of the abdomen, but not anasar¬
cous
, neither is any fluctuation perceptible:
flatulent and finds himself relieved by the
expulsion of wind. Urine rather decreased
in quantity
, the colour of it healthy. [Bowel?] regular;
fӕces of the natural consistence and colour: -
Thirst, but not remarkable: - a tolerable
appetite to every meal except breakfast: -
seldom prespires: - swellings do not subside
by an horizontal posture: - spirits low: -
Pulse natural. -

Has been subject to regular attacks
of the Gout from the age of six and thirty. -


Has always shortbreathed, and liable
to a violent palpitation of his heart of using
any considerable exercise, or indeed when at
the Desk, to which he was, some years ago,
much confined; he has had for many years
a pain in his right breast, for which he found
the exercise of pumping water a temporary
relief.


About 5 years since he was suddenly
attacked by an uncommon cramp in his left
hand
, which was succeeded by a violent convul¬
sion of every part
; a few hours after his
recovery, he was again seized in the same
manner, and it went in the same manner (
as the first); - after this he went to Bath
where he drank the water plentifully, had
no return of the convulsions, but found the
swelling of His legs come on, on this he

returned home, and has remained with little
variation, but with fits of the Gout, rather more
frequent than before, in a very relaxed state. -


He has generally rode out, when
the weather permitted, about once every day,
either in a carriage or on horse back, the last
he finds a difficulty in bearing, both from the
exposure to the air, and the exercise being too
much for his strength; - He thinks his
Cough and Dyspnœa are much increased from a
cold he took some days since, when on horseback,
in a shower.


Has been ever subject to Tremors
and spasms. -
