The Patient is aged 12ve. has never enjoyed perfect health since he had
the Meassles of a bad kind when about two Years of age, but
was always delicate, Complaining of headachs, pains in his
Stomach and Belly with several other Worm Symptoms, for
which worm Medicines were constantly used, and he has
passed some Worms every Year -- Last Winter & spring more
frequently, and for three or four Years before -- besides his other
Complaints he was subject to a kind of fainting fitts / preceeded
always by a headach / for a few Moments, which rendered him
insensible, so that he would fall down if not supported - these
fitts were not attended with any Convulsion, and he recovered
immediatly and was relieved of his headach as he was taken
up or supported -- Last April he took several dozes of Callo¬
mel
purged off with Rhubarb, and afterwards some bitters which
seemed to have a good Effect - for he continued free from all
Complaints till August following, except one day in the
Month of June that he was sick, faintish and Complained of
his headach till he passed along live Worm which relieved him.

In August having Catched Cold by wett feet a fishing, he
was seized with a Cough and Asthma, and some returns of the
fainting fitts, but a little more Callomel and Rhubarb with a
Milk Diet and rideing he recovered in a few weeks, & enjoyed
tollerable good health and spirits till about six weeks ago.
He then began to Complain of headachs, pains in his Stomach &
Belly reaching to reaching to Vomit and throwing up his food, together
with a Strangury -- Upon the first appearance of these
Symptoms he got a Vomit, some Rhubarb and Lime watter, but they
continued notwithstanding for ten days - when he was totally re¬
covered from the Strangury after getting an uncommonly Copious
fœtid stool -------- -- however the Complaints of his

head and Stomach, and the throwing up Continued - for which
repeated Vomits were thought necessary /which always brought
up Crudities and Phlegm / and some Dozes of Rhubarb, and he
also took some Dozes of Columba Root - he seemed always
easier for some little time after the Vomits till Saturday the
7th. /ten days ago/ when next morning in place of his ordinary
throwing up he was seised with uncommon violent reaching
and agitation of his whole body, particularly of his Arms
which seem to be Convulsed - attended with Crying and great
Noise, and a Lump in his throat, which nearly stops his
breath
, and which he thinks he feels coming up from
his Stomach, and could bring up with his fingers and accord¬
ingly he always thrusts them into his mouth for that pur¬
pose, and yet he never throws up anything but a Cough
spittle - and He thinks he feels the Lump fall suddenly
back again - these fitts lasts from one to two Minuts, and
sometimes three with very little interruption, and return from
15 to 18 20 times a day, and always without fail when he
swallows any thing - he is never troubled with them when he
warms in Bed at Night, but about an hour after he wakens
in the morning, and sleeps tollerably well - when the fitt goes off
he has avery quick respiration for a few seconds, a frequent
Pulse, and often belches a little wind - sometimes they work him
into a sweat, and avery low pulse - he is naturally of a
thin habit, but he is much emaceated and weakened by his
Illness - his appetite is irregular, sometimes a little sharp - at
other times much blunted, but eats very little at any time - he
is rather inclined to be Costive than otherwise - and his fitts
rather encrease in Violence and duration----------------
