Cullen is sorry to see his condition deteriorating, and recommends opium for the pain. He mentions that he would recommend a mercurial course in other circumstances, but considering the patient's 'weakly state' he will leave that decision to the 'Gentleman on the Spot' (which is in or near Inverness).
