For the Revd. Mr Gillespy.

Your present complaint is Piles & has no connection
with your former complaints. You have no fistula and
therefore need no operation.

You must avoid costiveness by taking gentle laxatives
whenever you need them. Flores sulphuris; or good
Castor oil. When your piles come down they must be as
soon as possible reduced, by a horizontal Posture & gently
keeping it up into the fundament.

I have nothing new to offer for your former complaint
