1. His age? 2. His temperament; atrabilious or sanguine.?
3. Manner of life before the attack; Temperate; sedentary and with
much application?


4. Exposed to any sudden emotion? or any accident to which in any way
the disease might be imported.


How long since first came on? Came it on suddenly or by degrees?
Was he affected for some time with melancholy? Did it appear at [first?]
with some delerium or false notions on some subjects? suddenly furious?
What turns since, of alternate fits of curiosity & melancholy? So fu¬
rious as to need straight waistcoat or such like?

6. State of his sleep, & its effects on his mind? Ever had Opiates, or
with what effects?


7. state of his appetite? what are observed to be the effects of diet?
His present Diet?


Tried bathing, cold or warm? with what effects.


What medicines employed? what effects? liable to costiveness? what medicines
