Friday June 14th. 1781 being Called to Mr William Waddell,
understood that about a forthnight before, he had gone with a friend
about six or seven Miles, Came home a little to late, had drunk
some punch, and as the servant said was a litle the worse of
it, Says that when got about half way home, he was taken
with a listlessness, not able to sit his horse, with a great pain of
his right side, from the Clavicle doun to the right Hypochonder

then Crossed over to his umbilicus before, and to his Spine
and doun the small of his back
behind, He went to bed, lay
four or five days, when I saw him he was in a strong fever,
the pain being very great, Pulse very full and hard. Thirst,
Tongue foul, and white, a very fetid breath, great heat, Deafness
which had been gradually coming on for some days
, no Stool
for 4 or 5 days
. appetite gone The full and hard pulse, great pain &c. induced
me to take away Eight ounces of blood
, which instantly
relieved him very much, even while the blood was running,
the oppression when mostly off
, the pain was much abated,
he felt his feet have a kindlier heat, no doubt from the Spasm
being relaxed
which prevented the free Circulation to the
Surface and Extremities
, Ordered him to omit taking whole
mustard seed
, which he had hithertoo taken as he never had
a Stool without it for a long time, having been ordered it when
in London, tho the Effects of it at this time would have been

dread full, yet I could scarcely perswade him to give it over,
Next day I gave him a vomit, which brought op a deal of viscid
roapy Stuff,
His breath was after that much sweeter, and appeared
to be getting fast better, the fever gradually going of tho the
pain Still Continued in a lesser degree, I was for applying a
Blister to his right Side,
with a Saline Mixture to take, and
particularly a Gentle antiphlogistic Purgative, to make his belly
Solube
, as he had now a little respite, he would be no means
do any more. I then proposed Calling a Physitian, which he would
not allow neither, next day having come doun stairs, and being thirsty
he said he drank a large draught of small beer, which unluckily
happened to have been turned a litle, it made him very sick,
he vomited much,
ever since his hearing gradually turned worse,
and likeways he began to have a numbness of his right leg which
I now understood he had had before sometime formerly, It
gradually increased so as in a few days he could not lift it, upon the
appearances of these formidable Symptoms he Consented that
I should Consult Dr. Elliot at London, as he had attended him
in his former Sickness, tho I told him the propriety of having one
on the spot, I wrote Sir John Elliot the case as above, when I
received the following directions, I should suppose it would be of
use, to Clear the Primæ viæ fully, and also to follow such a plan as
may remove any tendency to obstruction in the liver, or in the
mesenterie System.
. The Draught A. will accomplish the first

intention, and the use of the Mixture B. the Second, After that
a strengthening plan may be usefull, I apprehend that the
draught C. will answer this purpose better than any other,
and should the plan of bracing the fibres answer my wishes,
Bathing in the Sea would render every part of the treat¬
ment more effectual.-

Take two drachms each of Soluble Tartar, Daffy's Elixir and Rose Syrup Solution. A half ounce of Infusion of Common Seneka. Let a mixture be made. One dose which is to be received in the early morning and repeated until a third of the draught is depleted. The following day, repeat.

Take half an ounce of Soluble Tartar, one and a half ounces of Simple Cinnamon water, half a pound of spring water, and one ounce of Syrup of Violets. Mix. Take a quarter daily in the Morning, Noon, Evening and Night and repeat for Six or Eight Days.

When bringing together Peruvian Bark quicken {illeg} let this be given only when the bowel is loose enough and serve with a Lenitive Electuary.

Take two ounces of Decoction Peruvian Bark Decoction, ten drops of Chalybeate Wine, a drachm of Powdered Peruvian Bark and two drachms of rectified whisky. Mix.


Having received the above directions July 6th. at which time the
fever was quite gone
, so deaf that he did not even hear the
sound of his own voice
, very Costive, pain Still Continues
in the right hypochonder
, but no further up, Crosses over
still, behind, and before, has no use of the right limb, being quite
insensible
, to take the prescription A as directed.
July 7th. Physick did well, thinks himself rather better, pain
of his right Hypochonder not so bad
, no fever or thirst, leg rather
better, can walk some
, To take prescription B. as he thinks his
appetite is rather better to take a little Chicken broth bread bery
or such like


9th. Continues rather better, can walk better, has some appetite
more feeling in his leg, pain of the right Hypochonder quite
gone
, Complains of a pain in his right temple, imagines the
bone is a little fallen in. Says that ↑it↑ began first, the last night of
the year 1778 being a great Storm in the Thames, his vessell broke
loose (being Lieutennent of a man of war) and he stood all night

on Deck, quite wet, and dreadfully Cold, ever after he had that
pain and depression as he thought in his temple
, he had after
that a severe fit of sickness
, in which he was attended by
Sir John Elliot, he got over it, tho he never was so stout &
strong again, he came home, and seemed to be tolerably well,
when receiving a letter from the admiralty, he went up to
London and went out to the relief of Gibraltar with Admiral
Rodney as first Lietenant of the Terribl 1 went with him
to the West Indies, was in three engagements, in the last of which
he lost his hearing for 2 days, from the noise of the Guns,
but recovered it a litle quite again, then was at lenth
obliged to leave the service, and come home, from his from his left
Eye turning gradually
Paralytic, having no power either over
the lid or Eye itself, what has happened since you have seen
above.


10th. Much the same as yesterday-


12th. Hearing no better, leg continues rather better, very Costive,
bowels easy, having no pain in them. Repeat A.


13 Continues gradually better, Physick did well, has a tolerable
appetite, Deafness no better, Complains greatly of a Tinnitus
arium
, to Continue B.


16th. Continues Still turning Better, except the hearing which is
litle better,
To begin the use of the Bark, or C. and to take {illeg}
Lenitive Electuary f. s. ut Solvatur alvus


19th. Does not seem to agree with the bark, is feverish, Pulse quicker, and
harder
, appetite gone then his the other leg turning weaker,, hearing

no better, headach, the pain Striking from one Temple to
the other,
weather excessively warm,


21st. Still turning worse, gradually weaker, Pulse always quicker
in the morning, with much hardness
, Omit the use of the Bark
and return to the use of Prescription B, keeping the belly soluble
with Lenitive Electuary.


23d. A good deal better, feverishness gone again, Pulse soft, and full,
and doun to its usual Standard 76,
pain of the right Temple continues
a little
, Deafness no better, has had no Stool since 21st. not having
taken the Electuary Lenitive, To take it now.


28th. Continues rather better, has no fever, thinks a numbness is coming
on his other leg, the leg that was bad before being almost quite well,

has a good appetite, surface of the body Cold particularly the
Extremities
, has been using some Port wine frequently, which I
ordered him to omit, will take no more Medicines having taken
none for some days, had a stool yesterday, the numbness of
his
left leg began two days ago.


August 1 Says he is almost well, except the hearing which is no better,
--6th. hearing no better, thinks his sight is a litle impaired, I
observed him this day to be affected with the Cynic Spasm, his right
Cheek swelled a litle
, with pain in it his mouth being of Con¬
sequence draun to the other side
, I had some time before writen
Sir John Elliot, whether I might try the Deobstruent effects of
Mercury, thinking thinking there might be some serous Effusion,
within the Skull
and having got his Sanction for it, ordered

him to take his of the Common blew pill each night.


9th not much difference since the sixth, thinks he sees rather
better
, Cheek still swelled and painfull, Cynic spasm still continues
11th. Complains a little of thirst, a leady taste in his mouth, Mouth a litle
sore
, Cynic spasm not just so bad, pain of the Cheek still continues,
sight much better, appetite tolerably good, Puls at its ordinary
standard full and soft
, to take prescription A.


16th. Much better, thirst gone, Physick did well, numbness in his leg
much better
, can walk tolerably well, is Chearfull.


20th. Continues his pills, is a great deal better, Complains of nothing but want of
hearing
except th a small ↑pain↑ still continuing in his right Temple risus
sardonicus still continues
, Appetite good, no fever, says he always sweating
in bed
till within these four or five nights he has been rather Cold,
got on more Cloths, numbness of his legs almost quite gone, never has
a Stool
but as he procures it with Lenitive Electuary, walks a good deal
about now, to Continue the pills


24th. Much the same-


28th. Not so well for tuo days past, he eat tolerably at Dinner and
drunk tuo or three Glass of wine, and while he was doing so I
observed his face turning very red and florid, he rose from
table went to his oun room, sent for me, said he was sick at
Stomach and would go to bed, his pulse was frequent, full, and
hard
, he had risen that Morning too early, and notwithstanding
the Caution I had given him, had walked much among the
wet grass, and I have always observed any stimulus such as
the Port is to increase his disorder, his mouth has not been

affected since last Purgation, as he wont confine himself from
his oun way of living any longer, to Omit the pills


30th. Somewhat better today, slept well after he went to bed
on the 28th., has no stools but what are procured by Electuary Lenitive
hearing no better, vast tinnitusaurum, will not hear of sea
bathing which was much urged by Sir John Elliot., the
following I tryed myself, I applyed a large blister to the right side
of his head above the Ear
, and to use prescription B.


31st. Thinks himself a great deal better, can walk with still
greater freedom, thinks his sight much relieved, says he can
speak better as it was uneasy for him to speak before
Yet the Tinnitus Aurum not better nor the hearing or Cynic
Spasm
, Blister had run a vast deal fever gone again, not to
taste wine, to Continue B.


Septr. 4 Continues gradually better but no alteration as to his
hearing
I applyed a Blister to the other side of his head and gav
him some Nitrous powders
with Camphire


8th. Blister ran well, run a good deal, is free of fever, no thirst, appetite
good, no hearing, tinnitus aurum still continues, as does the cynic
spasm
, can walk tolerably well, some not fiery pimples broke
about his head
, got him to try the sea bathing, for three week
but without any good Effect the tinnitus aurum seemd to be rathe{illeg}
worse of ↑it↑, Yet he seemed to walk rather the better has continued
much the same since the above Septr. 8th. till this Novr. 12
